Surgery: Indiana Spine Hospital and North Meridian Surgery Center, 13219 N. Meridian St., Carmel.
Imaging: Bring a disc or have your MRI, CT, or X-rays sent ahead. Outside imaging is reviewed at the visit.
Insurance: Indiana Spine Group accepts most major plans. Confirm coverage and any referral requirement with the scheduler.
Patient portal: Messages, results, and forms through the Indiana Spine Group portal.
